When a Colorado girl checked into the Colorado Orthopaedic and Surgical Hospital to have surgery on May 15, 2009, she was nervous, but expected everything to be successful. After the surgery, a combination of hospital error and negligence caused the girl to die while recovering.

According to reports, the 25-year-old woman was killed when a nurse gave her the wrong dose of medicine in the wrong way, stopping her heart. When the woman's heart stopped, a crash-cart was brought to her bed, but confusion of its contents and layout made the situation more chaotic.

Health officials have now put the hospital on watch so that an investigation into the hospital and the wrongful death can be performed. So far, officials have found problems with the staffing and administrating of the hospital.
